The Foundation’s time frames for accepting applications have changed, effective immediately, beginning with the application submission period for Spring (May) 2012
For the Spring (May) meeting, applications are now accepted from December 8th to January 15th
For the Fall (November) meeting, applications are now accepted from June 8th to July 15th

  This option downloads a PDF file that must be returned via US Mail or Express Delivery service. To submit electronically you must use the online option above.

Request an application by mail
  Submit a written request to the Foundation, on the stationery of the Catholic organization that will apply, via US mail or fax. Upon eligibility determination, an application package will be sent to you via US Mail.
